30 AUGUST 2016 | GENEVA New guidelines for the treatment of three common sexually transmitted infections have been issued by the World Health Organization in response to the growing threat of antibiotic resistance.

Chlamydia, gonorrhoea and syphilis are all caused by bacteria and they are generally curable with antibiotics. However, these STIs often go undiagnosed and they are becoming more difficult to treat, with some antibiotics now failing as a result of misuse and overuse. It is estimated that, each year, 131 million people are infected with chlamydia, 78 million with gonorrhoea, and 5.6 million with syphilis.

Resistance of these STIs to the effect of antibiotics has increased rapidly in recent years and has reduced treatment options. Of the three STIs, gonorrhoea has developed the strongest resistance to antibiotics. Strains of multidrug-resistant gonorrhoea that do not respond to any available antibiotics have already been detected. Antibiotic resistance in chlamydia and syphilis, though less common, also exists, making prevention and prompt treatment critical.

The new recommendations are based on the latest available evidence on the most effective treatments for these three sexually transmitted infections.

What Are The Symptoms

Many women and most men do not have any symptoms of trich. But when you do have symptoms, they usually start within 1 week after you were infected.

In women, symptoms include:

• Changes in your vaginal discharge. You may notice a color or odor that isnât normal.
• Vaginal itching.
• Pain during urination or sex.

In men, symptoms include:

• A burning feeling when you urinate.

The time from contact with the trich parasite until you get symptoms can range from 5 to 28 days.footnote 1 This is called the incubation period. You can spread trich to others during this time and until you finish the prescribed medicine. You should avoid all sexual contact until you finish taking your medicine and the symptoms are gone.

How Is Trichomoniasis Treated

Getting tested for trichomoniasis can be done with either a urine sample or vaginal swab. After getting a diagnosis, it is important that sexual partners also be treated, in order to prevent reinfection .

Trichomoniasis can be quickly treated using a medication with both antibiotic and antiprotozoal functions . When taking these medications, it is important to abstain from alcohol ingestion for 24-72 hours, as this can cause negative interactions . It is possible to transmit trichomoniasis even while being treated. Stay away from sexual contact for at least 7-10 days until after completion of the full course of medicationâeven if symptoms have already gone away .

A follow-up appointment should be scheduled three months after treatment . Antimicrobial resistance is possible with trichomoniasis infections, so if symptoms persist after treatment, be sure to follow-up sooner with your healthcare provider .

More recent retrospective analyses, however, have failed to establish a link between metronidazole exposure during pregnancy and birth defects . This fact, coupled with the known risk of pregnancy complications associated with T. vaginalis infection, has led many clinicians and researchers to propose that the risk to the fetus from a maternal trichomoniasis is far greater than any risk related to metronidazole exposure. As such, a single 2-g dose of metronidazole is the treatment currently recommended by the Centers for Disease Control and Prevention for women at any stage of pregnancy. Alternatively, a daily intravaginal dose of 100 mg of clotrimazole for 6 days can provide temporary relief in the first trimester, followed by standard metronidazole therapy in the second trimester.
