Study Design And Setting

What is Azithromycin?

This is a double-blind RCT. Given our primary outcome is treatment efficacy, our trial is double blind to minimize bias that could arise as a result of the different dosing regimens of the two drugs . For example: i) it is possible that taking a 7-day course of daily doxycycline rather than a single dose of azithromycin may deter people from resuming sexual activity while taking treatment, thereby reducing their risk of a new infection, and ii) participants could be less adherent to a 7-day regimen which could impact efficacy . The trial will be conducted within sexual health clinics in Victoria and New South Wales in Australia and in accordance with the Declaration of Helsinki. The trial was approved by the Alfred Hospital Ethics Committee .

Does Azithromycin Work For Chlamydia

Azithromycin works to treat genital chlamydia in both men and women by stopping the bacteria from multiplying.

Studiessuggest that a one-gram dosage has 97% efficacy.

That means that for every 100 people who take azithromycin to treat chlamydia, 97 will be cured and three will not be cured.

In order to effectively treat chlamydial infections, azithromycin should be taken as prescribed and until the dosage is completed.

Ending the medication early increases the chance that the bacteria will not be completely killed off.

How Much To Take

The dose will depend on your infection.

The usual dose to treat Chlamydia is two 500 mg tablets taken as a single dose.

For other infections Zithromax is usually taken once a day. Sometimes the dose is taken once a week. Your doctor will decide the right dose for you.

Chlamydial Infection Among Neonates

Prenatal screening and treatment of pregnant women is the best method for preventing chlamydial infection among neonates. C. trachomatis infection of neonates results from perinatal exposure to the mothers infected cervix. Initial C. trachomatis neonatal infection involves the mucous membranes of the eye, oropharynx, urogenital tract, and rectum, although infection might be asymptomatic in these locations. Instead, C. trachomatis infection among neonates is most frequently recognized by conjunctivitis that develops 512 days after birth. C. trachomatis also can cause a subacute, afebrile pneumonia with onset at ages 13 months. Although C. trachomatis has been the most frequent identifiable infectious cause of ophthalmia neonatorum, neonatal chlamydial infections, including ophthalmia and pneumonia, have occurred less frequently since institution of widespread prenatal screening and treatment of pregnant women. Neonates born to mothers at high risk for chlamydial infection, with untreated chlamydia, or with no or unconfirmed prenatal care, are at high risk for infection. However, presumptive treatment of the neonate is not indicated because the efficacy of such treatment is unknown. Infants should be monitored to ensure prompt and age-appropriate treatment if symptoms develop. Processes should be in place to ensure communication between physicians and others caring for the mother and the newborn to ensure thorough monitoring of the newborn after birth.

What Are Side Effects Associated With Using Azithromycin

Side effects of azithromycin include:

  • Diarrhea
  • Low white blood cell count
  • Black, tarry stool
  • Azithromycin has serious interactions with at least 38 different drugs.
  • Azithromycin has moderate interactions with at least 144 different drugs.
  • Azithromycin has mild interactions with at least 38 different drugs.

    What Are The Treatments For Chlamydia

    If you are diagnosed with chlamydia, your doctor will prescribe oral antibiotics. A single dose of azithromycin or taking doxycycline twice daily for 7 to 14 days are the most common treatments and are the same for those with or without HIV.

    With treatment, the infection should clear up in about a week. Do not have sex for at least 7 days until you have taken all of your medication, and do not stop taking the antibiotics even if you feel better.

    Your doctor will also recommend that your partner be treated as well to prevent reinfection and further spread of the disease.

    Women with serious infections, such as pelvic inflammatory disease, may require a longer course of antibiotics or hospitalization for intravenous antibiotics. Some severe pelvic infections may require surgery in addition to antibiotic therapy.

    How Antibiotics For Chlamydia Are Administered

    Antibiotics for uncomplicated genital chlamydia infections are taken orally.

    • Where Azithromycin is used, it is given as a three day course. This will usually be two 500mg tablets on the first day, followed by one 500mg tablet on the second and third days.
    • If Doxycycline is used, NICE recommends 100mg taken twice a day for one week. So one dose might be one 100mg tablet, or two 50mg tablets.
    • Ofloxacin might be given as a once daily 400mg dose for one week, or a 200mg dose twice daily for one week.
    • Erythromycin for chlamydia might also be prescribed in one of two ways: as a 500mg dose taken four times daily for one week or as a 500mg dose taken twice a day for up to two weeks.
    • Amoxicillin is given as a 500mg dose , taken three times daily for one week.

    Is Treatment Always Necessary For Chlamydia

    How To Use Azithromycin Oral

    Take this medication by mouth as directed by your doctor, usually once daily with or without food. You may take this medication with food if stomach upset occurs. The dosage is based on your medical condition and response to treatment. For children, the dose is also based on weight.

    Shake the suspension well before using. Carefully measure the dose using a special measuring device/spoon. Do not use a household spoon because you may not get the correct dose. If your child vomits the 1-day treatment dose within 30 minutes, call your doctor to see if you need to repeat the dose.

    Continue to take this medication until the full prescribed amount is finished, even if symptoms disappear after a few days. Stopping the medication too early may allow bacteria to continue to grow, which may result in a return of the infection.

    Antacids containing aluminum or magnesium may decrease the absorption of azithromycin if taken at the same time. If you take an antacid that contains aluminum or magnesium, wait at least 2 hours before or after taking azithromycin.

    Summary Of The Evidence

    There is no evidence relating to patient values and preferences but the Guideline Development Group agreed that there is probably no variability in the values people place on the outcomes. Research related to other conditions indicates that adherence may be improved with simpler medication regimens. The GDG therefore agreed that azithromycin may be more acceptable to patients since it is a single dose regimen . There is little to no evidence for equity issues and feasibility. Resistance in other infections that often co-occur with chlamydia may restrict the use of some medicines, such as ofloxacin. For many of these medicines, costs may differ between countries in places with high incidence of chlamydia, the cost differences between azithromycin and doxycycline may be large due to greater numbers of people requiring treatment.

    In summary, there was moderate quality evidence for trivial differences in benefits and harms between azithromycin and doxycycline, and although the cost of azithromycin is higher, the single dose may make it more convenient to use than doxycycline. While the differences are also trivial with the other medicines, the evidence is low quality and these are therefore provided as alternatives, with the exception of delayed-release doxycycline, which is currently expensive.
