Clinical Specimens For Ct Testing

Diagnosis, treatment, and prevention of chlamydia | Infectious diseases | NCLEX-RN | Khan Academy

In principle, all relevant clinical materials can be analyzed by NAATs, including urethral, cervical, vulvo-vaginal, anorectal and ocular swabs, first void urine , sperm or tissues. FDA -cleared commercial NAATs are approved for first void urine, urethral and cervical swabs, and most of them also for vaginal swabs . Non-invasive specimens are preferred materials, in particular for screening of asymptomatic persons. FVU and urethral swabs from male patients are equivalent with respect to performance of CT NAATs. Collection of urine, however, is much better accepted and therefore the recommended sample type in men . As the concentration of chlamydia sharply decreases during urination, it is important to use the first portion of micturition . In contrast, the chlamydia concentration in women with urogenital infection is comparatively higher in genital swabs than in urine . A study analyzing urine, vaginal and cervical swabs taken simultaneously from asymptomatic women showed that the NAAT detection rate was highest in self-collected vaginal swabs . Therefore, vaginal swabs are the recommended sample type for women. Endocervical swabs may also be used, especially when a pelvic examination is indicated. FVU is less appropriate for CT testing in women, and when used for screening by NAAT, it should be considered that the detection rate is up to 10% lower compared to vaginal and endocervical swabs .

What Is A Chlamydia Infection

Chlamydia genital infection is a sexually transmitted disease caused by the bacteria Chlamydia trachomatis. In fact, C. trachomatis is the most common sexually transmitted bacterial infection in the world. Interestingly, about 70% of genital infections are associated with few or no symptoms at all.

Chlamydial infections in women are more likely to remain asymptomatic than in men . However, women are more likely to develop long-term complications.

How Do I Know If I Have Gonorrhea

Some men with gonorrhea may have no symptoms at all. However, men who do have symptoms, may have:

  • A burning sensation when urinating
  • Painful or swollen testicles .

Most women with gonorrhea do not have any symptoms. Even when a woman has symptoms, they are often mild and can be mistaken for a bladder or vaginal infection. Women with gonorrhea are at risk of developing serious complications from the infection, even if they dont have any symptoms.Symptoms in women can include:

  • Painful or burning sensation when urinating
  • Increased vaginal discharge
  • Vaginal bleeding between periods.

Rectal infections may either cause no symptoms or cause symptoms in both men and women that may include:

  • Painful bowel movements.

You should be examined by your doctor if you notice any of these symptoms or if your partner has an STD or symptoms of an STD, such as an unusual sore, a smelly discharge, burning when urinating, or bleeding between periods.

How Is Chlamydia Diagnosed

Diagnose chlamydia with nucleic acid amplification tests , cell culture, and other types of tests. NAATs are the most sensitive tests to use on easy-to-obtain specimens. This includes vaginal swabs or urine.43

To diagnose genital chlamydia in women using a NAAT, vaginal swabs are the optimal specimen. Urine is the specimen of choice for men. Urine is an effective alternative specimen type for women.43 Self-collected vaginal swab specimens perform as well as other approved specimens using NAATs.44 Patients may prefer self-collected vaginal swabs or urine-based screening to more invasive specimen collection.45 Adolescent girls may be good candidates for self-collected vaginal swab- or urine-based screening.

Diagnose rectal or pharyngeal infection by testing at the anatomic exposure site. While useful for these specimens, culture is not widely available. Additionally, NAATs have better sensitivity and specificity compared with culture for detecting C. trachomatis at non-genital sites.46-48 Most tests, including NAATs, are not FDA-cleared for use with rectal or pharyngeal swab specimens. NAATs have better sensitivity and specificity compared with culture for the detection of C. trachomatis at rectal sites.46-48 However, some laboratories have met set requirements and have validated NAAT testing on rectal and pharyngeal swab specimens.

Treatment Options For Feline Chlamydia

A variety of treatment options exist for feline chlamydia. Typically, a veterinarian will prescribe a form of antibiotics to combat the bacteria. Tetracycline or steroid-based antibiotics are commonly used to treat it. These products are applied topically or orally. For cats suffering from conjunctivitis, you may need to apply the ointment directly to the eyes. As with any drug that is administered directly into the eye, use extreme caution and care when giving your cat a dose of tetracycline.

Many upper respiratory infections caused by feline chlamydia result in dehydration. In this case, your vet may recommend a replacement fluid therapy program to rehydrate your cat.

For most cases of feline chlamydia, a 3-week antibiotic treatment program will cure all symptoms. However, if your cat suffers from an underlying virus or from chronic chlamydia, his symptoms may return shortly after discontinuing the antibiotics, or they may be unresponsive to drug treatments in general. In these cases, speak with a veterinarian about further treatment options.

How Common Is Chlamydia

CDC estimates that there were four million chlamydial infections in 2018.3 Chlamydia is also the most frequently reported bacterial sexually transmitted infection in the United States.4 It is difficult to account for many cases of chlamydia. Most people with the infection have no symptoms and do not seek testing. Chlamydia is most common among young people. Two-thirds of new chlamydial infections occur among youth aged 15-24 years.3 Estimates show that 1 in 20 sexually active young women aged 14-24 years has chlamydia.5

Disparities persist among racial and ethnic minority groups. In 2020, chlamydia rates for African Americans/Blacks were six times that of Whites.4 Chlamydia is also common among MSM. Among MSM screened for rectal chlamydial infection, positivity ranges from 3.0% to 10.5%.6,7 Among MSM screened for pharyngeal chlamydial infection, positivity has ranges from 0.5% to 2.3%.7.8

What Can Be Done To Address The Problem

How to Prevent STDs

Klausner says a three-way approach is needed:

  • Control the spread of new infections through prevention, screening, and treatment. However, there is much less funding for such efforts. The CDC says that more than half of state and local STD programs have seen budget cuts in years. âThe money taken out of budgets absolutely correlates with the increases in cases of STDs that weâre seeing,â Englund says.
  • Develop new antibiotics. Thereâs good news here: A new antibiotic now being tested has shown promising results in recent trials. When and if it will be effective and enter the market is unknown.
  • Develop tests to identify the best treatments. Klausnerâs lab has developed a test, now in use at UCLA Health System, that can tell physicians which antibiotic a particular case of gonorrhea will respond to. This has allowed them to successfully treat many cases of gonorrhea with older, less expensive antibiotics.
