Can Chlamydia Test Be Wrong

The Health Risks Of Chlamydia

Why A Positive STD Test May Be False – Sexually transmitted disease, Gonorrhea, Chlamydia

For up to 40 percent of infected women, untreated chlamydia can lead to pelvic inflammatory disease . PID effects include abdominal pain, fever, internal abscesses and long-lasting pelvic pain effects also include scarring of the fallopian tubes, which can cause infertility and increase the chance of potentially life-threatening ectopic or tubal pregnancies.

Men can develop scarring of the urethra, making urination difficult and occasionally causing infertility. Although rare, both sexes are at risk of a type of arthritis known as Reiter’s Syndrome that causes inflammation and swelling of the joints.

If a pregnant woman has chlamydia, her baby may be born prematurely, have eye infections or develop pneumonia.

Atesting During The Incubation Period

The reason being the incubation period of both Chlamydia and Gonorrhea. What happens is when you test for Gonorrhea within the 7 days of contraction, the test result may appear negative. As Gonorrhea is an asymptomatic disease, its testing during its incubation period can result in false negative test result. Similarly for Chlamydia, if tested within most results are accurate after 2 weeks but it can take up to 6 weeks.

False negative test results are just not limited to Chlamydia or Gonorrhea alone. Diseases like HIV too have a complicated window period of getting tested accurately. Refer the Incubation chart of the STD which states the suitable time to get tested with accurate results for STDs.

What Is A False Positive

A false positive is when a test result comes back positive, but the patient does not have an STD. This outcome is much rarer than a false negative, because it is a lot harder to detect something that is not there. It would usually be caused by an issue with the test itself.

If you take a test that offers a positive result, but you are highly uncertain of the accuracy, you should take a retest. There are a number of different kinds of STD tests that work in slightly different ways, whether its a different sample type or a different manufacturer. Retesting with a different type of test should offer you increased confidence in the results.

How Do You Test For Chlamydia

Nucleic Acid Amplification The three NAA tests described below work by finding the DNA of chlamydia bacteria. Because NAA tests search for the bacterias genetic material, it is very unlikely that a false-positive test result will occur. The incubation period for chlamydia is 1-5 days, so wait at least five days after potential chlamydia exposure before getting tested to ensure the most accurate results possible.

Urine samples

  • Testing via urine samples needs to consist of first-catch urine . Patients should not include more than the first-catch in the collection cup to avoid diluting the sample.
  • Patients should not urinate for at least one hour prior to providing a sample.
  • Female patients should not cleanse the labial area prior to providing the specimen.

Swab cultures

  • Neonates conjunctival swab

Why shouldnt you get tested for chlamydia via a blood test?

Enzyme-linked immunosorbent assay Antibodies, IgM

  • IgM antibodies are found mainly in the blood and lymph fluid they are the first antibody to be made by the body to fight a new infection.
  • This blood samples results should not be used as a diagnostic procedure without confirmation of the diagnosis by another medically established diagnostic product or procedure.

Enzyme immunoassay Antibodies, IgG

  • IgG antibodies are the most abundant type of antibody they are found in all body fluids and protect against bacterial and viral infections.

During And After The Test

Dengue rapid test

When you have a urine STD test, it is usually during a regular appointment. You will be asked to pee into a sample collection cup or tube. That tube or cup will be given to one of the medical staff.

Your urine sample will then be sent out to a lab or tested at the clinic. You can leave as soon as you are done giving the sample or when your appointment is finished.

What If I Don’t Want To Be Tested For All Four Stis

We appreciate that some people may not wish to be tested for one or more of the STIs we test for. In some regions we offer people a choice of tests . If you are not offered the opportunity to choose which STIs you would like to be tested for, we suggest you visit your nearest clinic instead. Remember, all of your results are confidential.

When To See A Doctor

STI tests are not always a standard part of a gynecological exam or regular health checkup. This is why anyone who might have chlamydia should seek out a test.

One symptom of chlamydia is a burning sensation when urinating. Anyone with this issue should contact a doctor, even if a home chlamydia test has returned a negative result.

If a home test returns a positive result, a person should contact their doctor to confirm it.

A home chlamydia test might involve collecting a urine sample or genital swab at home and sending the sample to a lab. Or, it might involve visiting a lab, where a healthcare professional collects the sample.

When And How To Test Again

Gersh recommends that people get tested once a year, or after every new partner whichever comes first!

Its also good practice to get site-specific STI tests before engaging in sex acts that incorporate a new part of the body.

For example, if you and your partner have never had anal sex and now want to, its a good move to get anal STI tested first.

To get tested, find an STI clinic that tests for all the STIs youre interested in getting tested for.

If you dont know which STIs you want to get tested for and are looking for a doctor to breakdown what STIs youre most at risk for given what sex acts are part of your sex life, its best to go to a clinic that *doesnt* specify one or two or three STIs in their online marketing.

Some clinics, for example, only test for HIV or only test for gonorrhea and chlamydia.

Key Question 2 How Effective Are Different Screening Strategies In Identifying Persons With Gonorrhea And Chlamydia

Can Chlamydia Be Detected In A Urine Test

Summary

No studies compared the effectiveness of different screening strategies for gonorrhea or chlamydia in asymptomatic persons or the effectiveness of sampling from various anatomical sites, cotesting for concurrent STIs, or using different screening intervals. Several studies of screening in high-risk groups have been published, but they did not meet inclusion criteria because they enrolled both symptomatic and asymptomatic persons, lacked comparison groups, or did not report relevant outcomes. An observational study in the Netherlands evaluated a risk prediction tool to identify persons with chlamydia in high-risk populations. However, the tool was not an accurate predictor, and its applicability to practice in the United States is unclear. Prior reviews did not directly address the effectiveness of different screening strategies, but rather summarized risk factors associated with gonococcal and chlamydial infections., An observational study comparing nine sets of selective screening criteria for chlamydial infection among women attending family planning and STI clinics in the United States indicated that age alone had similar or better sensitivity and specificity as more extensive criteria. In this study, nearly 80 percent of cases were identified when testing 50 percent of the population and using an age cutoff of 22 years or younger.

Wouldnt Testing Everyone Stop The Spread Of Genital Herpes

We dont know. There is no evidence that diagnosing genital herpes with a blood test in someone without symptoms would change their sexual behavior and stop the virus from spreading. In addition, without knowing the benefits of testing, the risk of shaming and stigmatizing people outweighs the potential benefits. For these reasons, testing everyone for herpes is not recommended at this time.

How To Know If You Have Chlamydia

Chlamydia is incredibly difficult to diagnose due to its symptomless nature for 70% of women and 50% of men. The only certain way to know that you have chlamydia is to get tested. If you do have symptoms then you will still need to be tested to effectively diagnose that the symptoms are caused by chlamydia. If youre sexually active, you should get tested regularly whether you have symptoms or not.

How Long Does It Take To Show Up In People With Penises

Positive Predictive Value and Test Results

Theres no significant difference in the amount of time it takes for chlamydia symptoms to show up for people with penises as compared to people with vulvas.

The only major difference in the time it takes for symptoms to show up among people of various sexes may be related to how often symptoms show up.

According to the Childrens National Health System, 90 percent of people with vulvas dont ever experience any physical symptoms, while 70 percent of people with penises never notice any symptoms.

This difference in who actually experiences symptoms between these two groups may have some effect on how long it takes for symptoms show up. But theres never been any definitive link between your sex and when your symptoms appear.

Genital Herpes Is Common Why Doesnt Cdc Recommend Testing Everyone For This Std

CDC does not recommend herpes testing for people without symptoms. This is because diagnosing genital herpes in someone without symptoms has not shown any change in their sexual behavior nor has it stopped the virus from spreading. Also, false positive test results are possible. Even if you do not have symptoms, you should talk openly and honestly about your sexual history with your doctor to find out if you should be tested for any STDs, including herpes.

Although CDC does not recommend that everyone get tested for herpes, herpes testing may be useful in some situations. Herpes blood tests might be useful

  • If you have genital symptoms that could be related to herpes,
  • If you have a sex partner with genital herpes, or
  • If you want a complete STD exam, especially if you have multiple sex partners.

Please note that while a herpes blood test can help determine if you have herpes infection, it will not be able to tell you who gave you the infection.

Is A Blood Test For Genital Herpes Included When I Ask To Be Tested For Everything Why Does Cdc Recommend Testing For Other Stds But Not Herpes

Herpes blood tests may or may not be included. Your doctor chooses STD tests based on your sexual behaviors , as well as how common the infection is in the area that you live in. This is why you should have an open and honest discussion with your doctor about your sex practices and history. When you go in for STD testing, it is important to ask your doctor which infections you are and are not being tested for, and why.

STD tests are usually done for infections that have serious outcomes if they are not treated. For example, finding and treating curable STDs like chlamydia can stop them from causing serious complications like infertility in women. Genital herpes does not usually result in serious outcomes in healthy, non-pregnant adults. More often, the stigma and shame from a genital herpes infection can be more troubling to someone who is infected than the disease itself. If you are worried about genital herpes, you should talk with your doctor about whether you should be tested.

What Can Cause False Results

The incubation period of chlamydia is normally between one to five days so, getting tested too early after a high-risk sexual encounter, may show a false negative result. Also, a positive result may reflect an old infection, in that case, we can call it a false positive since the results do not indicate an active infection.

The incubation period of gonorrhea is normally between two to six days however it can range anywhere from one to fourteen days. So, getting tested before that time will not give accurate results. It is always recommended to get tested after the incubation period has passed to prevent a false negative test, otherwise you would be advised to take the test again.

Limits Of Urine Tests For Gonorrhea And Chlamydia

Diagnosis, Treatment, and Prevention of Chlamydia

In 2018, 1.8 million cases of chlamydia were reported to the CDC in addition to 583,405 cases of gonorrhea. These numbers show an increment of 19% and 63% since 2014 for the two diseases respectively.

Most infections with gonorrhea and chlamydia are asymptomatic. The fact that many people have no symptoms means that the only way to detect and treat these infections is through screening.

In men, these diseases most commonly infect the urethra, and in women the cervix. However, it is possible to get both of these diseases in the throat, from oral sex. Anal sex can also lead to rectal chlamydia and rectal gonorrhea infections.

Neither rectal nor oral/throat infections will be detected by urine testing. It is therefore important to let your healthcare provider know if you have unprotected oral or anal sex. Testing should be done separately for those sites.

Currently, it is recommended that men who have sex with men undergo urine, throat, and anal screening once a year. Other people who regularly have unprotected oral or anal sex should consider a similar screening regimen. People who only engage in vaginal intercourse can get by with urine testing alone for gonorrhea and chlamydia.

A Delay Or No Treatment

A false negative test can be a complete disaster, especially when you have your STD in the advanced stage and you require immediate medical attention or treatment. Chlamydia and Gonorrhea both are asymptomatic diseases in their respective initial stages, meaning they dont show much symptoms or signs on the body during their initial stages. False Negative test result may end up a person thinking that he is not suffering from any diseases. Thus, doesnt require any medical attention.

What Is A Discordant Result

SH:24 uses highly sensitive & specific diagnostic tests for HIV, Syphilis, Chlamydia and Gonorrhoea however, when tens of thousands of tests are done each year it is possible for unexpected results to occur.

One reason for this is contamination, which can occur at the time of sampling or in the laboratory. The molecular amplification tests we use are very powerful and the smallest trace of the organism can result in a positive or reactive result. Since you are testing at home contamination is unlikely and our partner laboratory abide by strict quality controls to ensure that cross contamination does not occur consequently, it is highly unlikely for the test result for Chlamydia & Gonorrhoea to be wrong.

When SH:24 refers you to a service for treatment for Chlamydia & Gonorrhoea we do not expect the clinic to repeat the test at this time however, in some rare circumstances when the result does not match the clinical history the clinic you visit may opt to repeat the test and wait for that result.

What To Do After First False Positive Chlamydia Test Result

Chlamydia rapid test

We should try as much as possible never to accept a false positive test result without asking for a re-test especially when we are very sure of our status. Because accepting a false positive Chlamydia test result could lead to unethical and unnecessary surgery, treatment or inefficient/ineffective doctors appointment, when unknowingly to us and the lab technician we never did had such infection after all.
