Chlamydia is intracellular bacterium which makes the host cell parasitized for energy. They have cell walls with outer and inner membranes just like Gram-negative bacteria. They replicate by binary fission and contain RNA, DNA and ribosomes.

  • Pneumoniae: This is a significant respiratory pathogen found in humans and causes up to 10% of community acquired pneumonia cases. It is linked with acute respiratory diseases, asthma, pneumonia, bronchitis, acute chest syndrome, pharyngitis, coronary heart disease and Guillain-Barre syndrome.
  • Psittaci: This one infects a wide and diverse range of host species from birds and molluscs to mammals and causes severe pneumonia. This is seen worldwide as it is a common cause of atypical pneumonia. It is usually uncomplicated but in severe cases can cause disease affecting several organ systems especially in epidemic cases.
  • Counselling And Behavioural Approaches

    Counselling and behavioural interventions offer primary prevention against STIs , as well as against unintended pregnancies. These include:

    • comprehensive sexuality education, STI and HIV pre- and post-test counselling
    • safer sex/risk-reduction counselling, condom promotion
    • STI interventions targeted to key populations, such as sex workers, men who have sex with men and people who inject drugs and
    • STI prevention education and counselling tailored to the needs of adolescents.

    In addition, counselling can improve peoples ability to recognize the symptoms of STIs and increase the likelihood they will seek care or encourage a sexual partner to do so. Unfortunately, lack of public awareness, lack of training of health workers, and long-standing, widespread stigma around STIs remain barriers to greater and more effective use of these interventions.

    What Happens If Chlamydia Is Untreated

    In the long term, if chlamydia is untreated, the infection may spread across the reproductive system and other organs. Inflammation of the cervix causes odorous vaginal discharge, abnormal bleeding, and pain during intercourse. With more widespread infection involving the Fallopian tubes and uterus, a condition called pelvic inflammatory disease can develop.6

    The chlamydia bacteria infects the Fallopian tubes causing swelling and scarring of the tissues. This leads to blockage, and prevents the movement of eggs into the uterus. For this reason, fertilisation becomes a problem, and chlamydia can be responsible for causing infertility.7 Other sexually transmitted diseases, such as gonorrhoea, may also cause PID in the same way.

    Scarring of the Fallopian tubes can increase the chance of ectopic pregnancy, where the fertilised egg implants somewhere other than the uterus, for example in the Fallopian tubes. When this occurs, the embryo begins to grow and the tubes can rupture, leading to sudden onset of severe pain and bleeding. This can be life threatening if the embryo is not surgically removed. Therefore, it is important to seek immediate medical attention if you have sudden severe tummy pain with a positive pregnancy test.

    Chlamydia can be passed from pregnant women to their unborn child. As such, it is associated with pre-term births, infant eye infections , and lung infections . For this reason, it is important to receive treatment for chlamydia early.

    What Happens If I Dont Get Treated

    The initial damage that chlamydia causes often goes unnoticed. However, chlamydia can lead to serious health problems.

    If you are a woman, untreated chlamydia can spread to your uterus and fallopian tubes . This can cause pelvic inflammatory disease . PID often has no symptoms, however some women may have abdominal and pelvic pain. Even if it doesnt cause symptoms initially, PID can cause permanent damage to your reproductive system. PID can lead to long-term pelvic pain, inability to get pregnant, and potentially deadly ectopic pregnancy .

    Men rarely have health problems linked to chlamydia. Infection sometimes spreads to the tube that carries sperm from the testicles, causing pain and fever. Rarely, chlamydia can prevent a man from being able to have children.

    This is Part I of a two-part article on drug treatment of common sexually transmitted diseases. Part II, Vaginal Infections, Pelvic Inflammatory Disease and Genital Warts, will appear in the next issue of AFP.

    Several advances have been made in the treatment of sexually transmitted diseases . These advances have been incorporated into the 1998 Guidelines for the Treatment of Sexually Transmitted Diseases, published by the Centers for Disease Control and Prevention .1

    Highly effective single-dose oral therapies are now available for most common curable STDs. Single-dose regimens may be used for the treatment of chancroid, nongonococcal urethritis, uncomplicated gonococcal infections, bacterial vaginosis, trichomoniasis, candidal vaginitis and chlamydial infections.

    Improved therapies are now available for the treatment of genital herpes and human papillomavirus infections. New regimens have been approved for the use of acyclovir in the treatment of genital herpes. In addition, two new antiviral agents, valacyclovir and famciclovir , have been labeled for the treatment of genital herpes. Patient-applied therapies are now recommended for management of HPV.
