Antibiotics For Treating Genital Chlamydia Trachomatis Infection In Men And Non

Review question

This systematic review assessed the effectiveness and safety of antibiotic treatment for Chlamydia trachomatis genital infection in terms of microbiological or clinical failure in men and non-pregnant women.

Background

CT is the most frequent cause of urinary tract and genital infections in women and men. However, women frequently show no symptoms when they are infected. CT infection can lead to complications or cause further problems in reproductive health in women , and men , or chronic pelvic pain. Clinical guidelines for treating CT do not recommend a preferred antibiotic treatment. This Cochrane review evaluates all randomised controlled studies , that included antibiotics for the treatment of genital CT infection that are recommended by the most up-to-date clinical guidelines.

Search date

We searched for studies published up to June 2018 that provided information about failure to eliminate the CT infection or improve the symptoms, presence of adverse events, antimicrobial resistance, and reinfection. as treatment outcomes

Study characteristics

Study funding sources

One study reported funding from academic grants, another four studies declared having received sponsorship or grants from pharmaceutical companies. The other studies declared that they were self-funded or did not mention funding at all.

In men, chlamydial infection of the lower genital tract causes urethritis and, on occasion, epididymitis. Urethritis is secondary to C. trachomatis infection in approximately 15 to 55 percent of men, although the prevalence is lower among older men.2 Symptoms, if present, include a mild to moderate, clear to white urethral discharge. This is best observed in the morning, before the patient voids. To observe the discharge, the penis may need to be milked by applying pressure from the base of the penis to the glans.

The diagnosis of nongonococcal urethritis can be confirmed by the presence of a mucopurulent discharge from the penis, a Gram stain of the discharge with more than five white blood cells per oil-immersion field, and no intracellular gram-negative diplococci.2 A positive result on a leukocyte esterase test of first-void urine or a microscopic examination of first-void urine showing 10 or more white blood cells per high-powered field also confirms the diagnosis of urethritis.

Where Can I Get A Test

What are the Most Common Reactions to Amoxicillin?

There are a number of services you can go to. Choose the one you feel most comfortable with.

A chlamydia test can be done at:

  • a genitourinary medicine or sexual health clinic
  • your general practice
  • contraception and young peoples clinics
  • some pharmacies.

Abortion clinics, antenatal services and some gynaecology services may also offer a chlamydia test.

In England, if youre a woman aged under 25 years old, you may be offered a chlamydia test as part of the National Chlamydia Screening Programme when you visit some service for other reasons, for example at a pharmacy or your GP.

The NCSP aims to identify people without symptoms to reduce the complications of untreated infection. If chlamydia is not treated, it can cause health complications, especially in women. Untreated chlamydia in women can cause pain in the pelvis, ectopic pregnancy and infertility .

If you are a woman aged under 25 years old and you are offered a chlamydia test as part of the NCSP you should consider taking it.

In many areas, free home self-sampling tests for chlamydia are available to order online. This is where you take your own sample and send it to be tested. See www.nhs.uk

Its also possible to buy a chlamydia test to do at home. The accuracy of these tests varies. Some types are very accurate when carried out according to the instructions, others can be less reliable. If you buy a testing kit make sure you get advice from a pharmacist or your doctor.

Do Utis Need Treatment With Antibiotics

Currently, guidelines advise that even women with uncomplicated UTIs are offered antibiotics, depending on the severity of symptoms.5 If you only have mild symptoms, you can try waiting for up to 48 hours to see if the infection settles down by itself, for example, if it is not convenient for you to see a doctor. If you do visit a doctor and your symptoms are mild, you may be given a delayed prescription to collect antibiotics if your symptoms start getting worse, or do not improve.6

If you do decide to wait and see if things settle down, you can try taking painkillers like paracetamol and ibuprofen, and you are encouraged to keep well hydrated. Drinking plenty of water is easier said than done when you have a UTI, as there is a temptation to drink less seeing as you will be feeling the urge to pee all the time. However, good levels of hydration do help to fight the infection.7

Currently, guidelines advise that even women with uncomplicated UTIs are offered antibiotics. However, if you only have mild symptoms, you can try waiting for up to 48 hours to see if the infection settles down by itself

Should your symptoms be causing you distress, or you find they dont disappear, or even worsen, after 48 hours, you should go and see a doctor, who will prescribe antibiotics if a UTI is confirmed. There are lots of different antibiotics used to treat UTIs, and again this choice can depend on whether you have a complicated UTI or not.9

How Accurate Are The Tests

What You Need to Know About Chlamydia

The accuracy of a chlamydia test depends on the kind of test used and the type of sample thats collected. The recommended tests are over 95% accurate in picking up chlamydia. As no test is 100% accurate theres a small chance that the test will give a negative result when you do have chlamydia. This is known as a false negative result. This can sometimes explain why you might get a different result from another test or why you and a partner might get a different test result.

Its possible for the test to be positive if you havent got chlamydia, but this is rare.

Similarities And Differences Between Utis And Stds

UTIs share symptoms similar to STDs and are misdiagnosed more often than you may think. According to the American Society for Microbiology, 64 percent of the patients with a sexually transmitted infection were actually diagnosed as having a UTI instead. This is problematic. Not only are women being needlessly prescribed a UTI treatment , but it may also cause an undiagnosed STD to develop into a more serious issue. In the beginning stages, many STDs are treatable, but the further they are allowed to progress, the more complicated and expensive treatment becomes.

Is It Possible To Prevent Urinary Tract Infections With Diet And Supplements

It is possible to reduce the chance that a UTI will develop with dietary methods and some supplements but prevention of all UTIs is unlikely with these methods. Supplements such as eating cranberries, taking vitamin C tablets, and eating yogurt and other substances also may reduce the chance that a UTI will develop . However, as stated in the prevention section, changes in a personâs lifestyle may reduce the chance getting a UTI as good as, if not better than, any diet or supplement.

A Urinary Tract Infection :

It is very common for women of all ages to get infections in their urethrathe tube that carries urine from your bladder to the porcelain container. UTIs happen when bacteria or other agents like viruses or fungi get into your urethra, bladder or kidneysusually starting in your urethraand cause an infection. This can happen by wiping the wrong way, or wearing thongs that transfer bacteria from the anus to the urethra .

Sex can cause a UTI, because sexual intercourse can help push bacteria into the urethra. But having sex doesnt guarantee that you will get a UTI. There are preventative measures that can be taken when engaging in sexual activity that can help reduce your risk of getting a UTI.

  • Pee before and after sex
  • Stay hydrated to help rid your urinary tract of bacteria
  • Avoid using spermicide as a form of contraceptive, as it may disrupt the balance of normal bacteria in the vagina and urethra1

The most common symptoms of UTIs are urgency to urinate, feeling the need to urinate very frequently and pain or burning when you pee. UTIs can usually be fixed pretty quickly with antibiotics from your primary care provider.

Now, while it isnt invariable that sex led to your UTI, it is necessary for you to have sex to get an STI. Viral infections need other strategies.

Does Amoxicillin Treat Bacterial Vag

Common side effects of chlamydia antibiotics

Amoxicillin may help to a certain extent, but is not totally useful for treatment of bacterial vaginitis which occurs due to an increase in the number of several types of anaerobic bacteria that are normally present in the vagina. Metronidazole seems to be the most effective treatment for anaerobic bacteria.

How Do I Know If I Have Chlamydia

If you suspect you have chlamydia, your doctor may want to test cervical or penile discharge or urine using one of several available methods.

In most cases of chlamydia, the cure rate is 95%. However, because many women don’t know they have the disease until it has caused serious complications such as pelvic inflammatory disease, sexually active women under age 25 and others at higher risk should be tested for chlamydia once a year during their annual pelvic exam even if they dont have symptoms.

Pregnant women should also be tested as part of their routine lab work.

Why Early Detection Is Necessary

When symptoms reach a stage that patients end up in the emergency room, those with undiagnosed STIs are often misdiagnosed. In fact, studies show that almost two thirds of them are diagnosed with and treated for a UTI. This will not clear up chlamydia, since its a short course of antibiotics that are typically used.

Because its left undiagnosed and untreated, chlamydia often involves serious complications that are life altering. Because it resides in the mucusy parts of the body, chlamydia can affect not just the sexual organs but also the anus, the eyelids, and the throat. One of the most common complications of untreated chlamydia is pelvic inflammatory disease, or PID, which has its own set of concerns, including complications during pregnancy. Other problems include:

  • Pain and inflammation in the sexual organs, as well as in the prostate in men
  • Damage to the tear ducts and the cornea
  • Infertility in both men and women

Can You Get A Uti From Fingers

Its pretty easy to get a urinary tract infection. Bacteria that live in the vagina, genital, and anal areas may enter the urethra, travel to the bladder, and cause an infection. This can happen during sexual activity when bacteria from your partners genitals, anus, fingers, or sex toys gets pushed into your urethra.

What Happens If An Antibiotic Doesn’t Work For A Urinary Tract Infection

Chlamydia Infection – HealthExpress

Antibiotics are the mainstay of treatment for urinary tract infections , most of which are caused by a bacteria called Escherichia Coli . Infections of the lower urinary tract, which includes bladder infections , are the most common type of UTI and are usually treated with a 3-5 day course of antibiotics. Sometimes, however, the antibiotic prescribed to treat a bladder infection doesnt work.

If you suspect your antibiotic isnt working you should promptly contact your healthcare provider. Left untreated a UTI may become more serious and in rare cases cause permanent or life-threatening complications.

So How Can You Tell If Youre Suffering Due To A Uti Or An Sti

Its difficult to differentiate a UTI from an STI based on symptoms alone, since both infections cause burning during urination, pelvic pain, and a frequent, sudden urge to urinate. However, if you are sexually active and are also experiencing vaginal symptoms such as discharge, bleeding and/or irregular periods, it may suggest an STI. Make sure to talk to your doctor about your specific symptoms and risk factors for these types of infections.

The only way to determine exactly what kind of infection you are dealing with is to head to the doctor for testing.

Do Hot Tubs Cause Utis

Though time in the hot tub is often blamed for urinary tract infections, it is actually not a common cause of UTI and very rare that a long soak would cause the infection. Hot tubs, or even very long baths, can cause the spread of yeast, known as vaginitis, and common skin infections like folliculitis. In the rare instance that a UTI is contracted while in the hot tub the bacteria known as, pseudomonas are going to be the cause. This bacteria makes up less than thirty percent of UTI cases, as E. coli is the most common cause of cases.

Both infections present with similar symptoms and irritation so the one might be confused for the other and a doctor should be consulted in either case in order to gain a correct diagnosis and the best treatment. Even though time in the hot tub is thought to be a major cause of UTIs, women are more likely to contract the bacteria after intercourse, or through the use of diaphragms.

Urogenital Infection In Women

In women, chlamydial infection of the lower genital tract occurs in the endocervix. It can cause an odorless, mucoid vaginal discharge, typically with no external pruritus, although many women have minimal or no symptoms.2 An ascending infection can result in pelvic inflammatory disease .

Physical findings of urogenital chlamydial infection in women include cervicitis with a yellow or cloudy mucoid discharge from the os. The cervix tends to bleed easily when rubbed with a polyester swab or scraped with a spatula. Chlamydial infection cannot be distinguished from other urogenital infections by symptoms alone. Clinical microscopy and the amine test can be used to help differentiate chlamydial infection from other lower genital tract infections such as urinary tract infection, bacterial vaginosis, and trichomoniasis.3 In addition, chlamydial infection in the lower genital tract does not cause vaginitis thus, if vaginal findings are present, they usually indicate a different diagnosis or a coinfection.

Some women with C. trachomatis infection develop urethritis symptoms may consist of dysuria without frequency or urgency. A urethral discharge can be elicited by compressing the urethra during the pelvic examination. Urinalysis usually will show more than five white blood cells per high-powered field, but urethral cultures generally are negative.

Seek Medical Attention For Utis

Chlamydia Treatment and Prevention

It is important to seek medical attention if you think you may have a UTI particularly if you think you may have a bladder or kidney infection, both of which are very serious conditions. Early treatment of urinary infection can help to prevent infection spreading to the bladder or kidneys.

Your doctor will test your urine to check which micro-organism is present. Urinary tract infections usually respond quickly and well to antibiotics.
