Why Is Azithromycin Not A Chlamydia Treatment Anymore

The British Association for Sexual Health and HIV recently released guidelines explaining that Azithromycin is no longer an effective treatment for chlamydia. This is because of increasing levels of bacterial resistance to azithromycin. Instead, the recommended first-line treatment is now only doxycycline.

How Is Chlamydia Diagnosed

The most common test for chlamydia is called a nucleic acid amplification test . Your provider takes a sample of fluid by doing a vaginal/cervical swab or collecting a urine sample. Then, they send the sample off to a lab to check for the bacteria that causes chlamydia. Your provider may do the test in an office, or they may ask you to do an at-home chlamydia test. Follow your providers instructions carefully to ensure you get accurate test results.

Because most chlamydia cases are asymptomatic, its important to get screened for chlamydia even if you dont notice any signs of infection. The CDC recommends that sexually active cisgender women who are high-risk for chlamydia get screened regularly. Women, more so than men, experience the most severe complications from chlamydia. Transgender men and nonbinary individuals with vaginas should be screened regularly, too, as they can experience the same complications of chlamydia.

Youre considered high-risk if you:

  • Have had chlamydia infections previously.

Cisgender men, or trans and nonbinary individuals with penises, should be screened for chlamydia if:

  • They live in a setting where chlamydia spreads frequently, like correctional facilities, adolescent clinics and sexual health clinics.
  • They have sex with other men.

  • Pelvic inflammatory disease . PID is a serious condition that requires hospitalization. It can occur when an untreated STI, like chlamydia, damages your reproductive organs. PID can lead to infertility and chronic pelvic pain. It can also cause an ectopic pregnancy, which is life-threatening for the fetus and potentially deadly for the mother or gestational parent, too.
  • Pregnancy complications. An untreated infection can lead to pre-term delivery. Also, if youre pregnant and have chlamydia, you can pass the infection on to your newborn. Babies born with chlamydia may have pneumonia or conjunctivitis that could lead to blindness if not treated.

Current guidelines both in Australia and internationally recommend testing people with symptoms, or sexual contacts of known cases. They dont recommend doctors screen people without symptoms.

When you screen, you have to be confident you have access to highly effective treatments, the treatments do not cause more harm than the condition itself and you have a good understanding of how often the condition progresses to cause complications.

Not only do many antibiotics have side-effects, they affect the bacteria in peoples gut. These bacteria are important to keep us healthy, and if we bombard them with antibiotics it can affect our health and also lead to antibiotic resistance in a whole range of other bacteria, not just MG.

Do I Need To See A Doctor

The answer is nearly always, yes. Whilst the irritation may not seem serious enough to warrant a visit to the GP, it is important to get checked. Many of these symptoms are common across a variety of conditions, and so its important to get a proper diagnosis. Its also likely that conditions left untreated get worse the longer you wait, the harder treatment can be. You also want to ensure you do not pass on any infections to sexual partners.

Its impossible to self-diagnose in these circumstances, given the range of causes. How Is Penile Discharge Treated

In our experience, the most common cause of penile discharge is STIs. Therefore the treatment in the form of antibiotics is selected on the basis of STI test results.

Recommendations on the choice as well as the dose of antibiotics is an area of specialised practice, and guided by current sensitivity patterns of various infections. For example, gonorrhoea is well known to develop resistance against the recommended antibiotics.

Presence of pain in testicles, genitalia or perineum in the patients with penile discharge indicates possible local spread of the infection, and thus longer courses of antibiotics are prescribed in such cases.

Those with discharge as a result of Non-specific urethritis are treated with the same course of antibiotics which treat discharge related to Chlamydia infection.

This is why it is essential to first collect the urine specimen and identify the cause before administering antibiotics. However, there may be times where you clinician recommends that you initiate treatment before your results come back .

If the cause of penile discharge is not an STI, we can also offer oral antibiotics/antifungals to treat the discharge.
