Chlamydial infection is the most frequently reported bacterial infectious disease in the United States, and prevalence is highest among persons aged 24 years . Multiple sequelae can result from C. trachomatis infection among women, the most serious of which include PID, ectopic pregnancy, and infertility. Certain women who receive a diagnosis of uncomplicated cervical infection already have subclinical upper genital tract infection.
Asymptomatic infection is common among both men and women. To detect chlamydial infection, health care providers frequently rely on screening tests. Annual screening of all sexually active women aged < 25 years is recommended, as is screening of older women at increased risk for infection . In a community-based cohort of female college students, incident chlamydial infection was also associated with BV and high-risk HPV infection . Although chlamydia incidence might be higher among certain women aged 25 years in certain communities, overall, the largest proportion of infection is among women aged < 25 years .

Summary Of The Evidence
There is no evidence relating to patient values and preferences but the Guideline Development Group agreed that there is probably no variability in the values people place on the outcomes. Research related to other conditions indicates that adherence may be improved with simpler medication regimens. The GDG therefore agreed that azithromycin may be more acceptable to patients since it is a single dose regimen . There is little to no evidence for equity issues and feasibility. Resistance in other infections that often co-occur with chlamydia may restrict the use of some medicines, such as ofloxacin. For many of these medicines, costs may differ between countries in places with high incidence of chlamydia, the cost differences between azithromycin and doxycycline may be large due to greater numbers of people requiring treatment.
In summary, there was moderate quality evidence for trivial differences in benefits and harms between azithromycin and doxycycline, and although the cost of azithromycin is higher, the single dose may make it more convenient to use than doxycycline. While the differences are also trivial with the other medicines, the evidence is low quality and these are therefore provided as alternatives, with the exception of delayed-release doxycycline, which is currently expensive.

Are Azithromycin And Doxycycline Safe To Use While Pregnant Or Breastfeeding
Doxycycline
- Tetracycline antibiotics, such as doxycycline, can have toxic effects on development of bone in the fetus. Therefore, tetracyclines are not recommended during pregnancy unless there is no other appropriate antibiotic.
- Doxycycline is secreted into breast milk but the extent of absorption by the breastfed infant is not known. Since tetracyclines can cause toxic effects on bone, the use of tetracyclines in nursing mothers is of concern. The physician must decide whether to recommend that a nursing mother discontinue nursing during treatment with tetracyclines or change to a different antibiotic.
Azithromycin
- There are no adequate studies of azithromycin in pregnant women. Azithromycin should only be used during pregnancy if it is clearly necessary.
- It is not known if azithromycin is secreted in breast milk.
Things To Be Careful Of
Protect your skin when you are in the sun, especially between 10am and 3pm. Do not use a sunlamp while taking doxycycline.
Doxycycline may cause your skin to be much more sensitive to sunlight than it is normally. Exposure to sunlight may cause a skin rash, itching, redness, or a severe sunburn.
If outdoors, wear protective clothing and use a 30+ sunscreen.
If your skin does appear to be burning, tell your doctor as soon as possible.
Be careful while driving or operating machinery until you know how doxycycline affects you.

Study Design And Setting
This is a double-blind RCT. Given our primary outcome is treatment efficacy, our trial is double blind to minimize bias that could arise as a result of the different dosing regimens of the two drugs . For example: i) it is possible that taking a 7-day course of daily doxycycline rather than a single dose of azithromycin may deter people from resuming sexual activity while taking treatment, thereby reducing their risk of a new infection, and ii) participants could be less adherent to a 7-day regimen which could impact efficacy . The trial will be conducted within sexual health clinics in Victoria and New South Wales in Australia and in accordance with the Declaration of Helsinki. The trial was approved by the Alfred Hospital Ethics Committee .
What Happens When You Mix Doxycycline With A Medication
Medicines that interact with doxycycline may either decrease its effect, affect how long it works for, increase side effects, or have less of an effect when taken with doxycycline. What If I Forget To Take It

Doxycycline Resistance And M Genitalium
Information on antimicrobial susceptibility of M. genitalium is scarce because of the limited number of strains isolated from clinical samples. Studies reporting MICs of doxycycline usually found low MIC values, i.e. 5 strains with MICs ranging from0.008 to 0.031 mg/L and 14 strains with MICs ranging from 0.06 to 0.12 mg/L . However, an in vitro antimicrobial susceptibility testing study conducted using both broth dilution and quantitative PCR showed an MIC range of 0.0631 mg/L indicating that the strains displayed reduced susceptibility to doxycycline but that these isolates remained rare. Finally, a recent larger study showed that 2 isolates out of 103 displayed MIC> 8 mg/L while for other isolates, MICs ranged from< 0.125 to 2 mg/L. However, doxycycline MICs did not correlate with treatment outcomes in this study. As far as molecular detection of mutations mediating resistance is concerned, macrolides and fluoroquinolones were mainly studied and to our knowledge, tetracycline resistance-associated mutations have not so far been identified in M. genitalium. Altogether, MICs mostly indicated susceptibility of M. genitalium to doxycycline and the rare isolates with reduced susceptibility cannot explain the poor efficacy of doxycycline in the treatment of M. genitalium infections., Considering the emergence of MDR M. genitalium strains, it thus appears important to elucidate reasons other than poor patient compliancefor the poor efficacy of doxycycline.

Is Azithromycin Better Than Doxycycline At Curing Chlamydia
STD guidelines still favor azithromycin over doxycycline for the treatment of chlamydia. This is because of the following reasons:
- Cure rates of azithromycin and doxycycline are similar, 97% and 98-100% respectively, according to a meta-analysis of 12 trials.
- Azithromycin is given as a single dose, doxycycline needs to be given for seven days, either as a once-daily or twice-daily dose.
- The dose of azithromycin can be easily supervised if need be it is much harder to supervise seven days of once daily or twice daily doxycycline treatment
- People are more likely to take a single dose of azithromycin than finish a seven-day course of doxycycline.
Recently, some reports have suggested that doxycycline may be more effective than azithromycin, particularly when medication adherence can be assured. There is also some concern that azithromycin may not be as effective for anogenital chlamydial infections.
Who Could Benefit From Using Antibiotics For Sti Prophylaxis
People at the highest risk of contracting bacterial STIs, especially those who may be at risk for repeated infections, may benefit most from taking STI prophylaxis. If you do not use condoms consistently, or at all, have multiple sexual partners and have had bacterial STIs in the past, you may be interested in the potential of this approach.
Bacterial STI prophylaxis can be taken regardless of HIV status. Many individuals who are HIV negative and on HIV PrEP use condoms less frequently or have stopped using them altogether. Similarly, some who are living with HIV also choose not to use condoms all of the time as an undetectable viral load prevents HIV transmission.
An accepted public health approach promotes the control of STIs among those at highest risk as a way of reducing STIs in the general population. An Australian modelling study supports this notion: it estimated that if half of Australian gay men took doxycycline as PrEP, and it was 70% effective against syphilis, then rates of syphilis would decrease by 50% after a year and 85% after a decade. Interestingly, the same finding applied if only 50% of the highest-risk group were taking doxycycline. This indicates that targeted interventions could have widespread community-level benefits.
Surveys in Australia, the US and the UK show high levels of interest among gay men and some healthcare providers in using doxycycline to prevent STIs, with some men already using this approach.

What Does Doxycycline Do For Cats
As with most antibiotics, doxycycline has good levels of efficacy against certain bacteria, while others may be resistant or just not susceptible to it.
For example, an upper respiratory infection in a cat caused by bacteria including Bordetella, Chlamidophila , or Mycoplasma will typically be susceptible, while a urinary tract infection caused by Proteus is unlikely to be.

Doxycycline is a tetracycline antibiotic. Tetracyclines are generally time-dependent bacteriostatic antibiotics. This means that the length of time they are employed against bacteria susceptible to them is most important.
Although termed bacteriostatic, implying they only inhibit the growth of bacteria, antibiotics like doxycycline do actively kill bacteria. They simply require a higher concentration than an antibiotic labeled as bactericidal. Management Of Sex Partners
Sex partners should be referred for evaluation, testing, and presumptive treatment if they had sexual contact with the partner during the 60 days preceding the patients onset of symptoms or chlamydia diagnosis. Although the exposure intervals defining identification of sex partners at risk are based on limited data, the most recent sex partner should be evaluated and treated, even if the time of the last sexual contact was > 60 days before symptom onset or diagnosis.
How Do I Take Doxycycline
For children weighing 76 pounds or more and adults aged 18 years or older:
- Take 1 pill in the morning with a full glass of water and
- Take 1 pill in the evening with a full glass of water .
The morning and evening doses should be taken 12 hours apart each day for as long as directed.
Doxycycline works just as well whether you take it with or without food or milk.

For children weighing less than 76 pounds , the dose is determined based on weight:

If you get an upset stomach when you take the medicine, take it with food.
Keep doxycycline away from children and pets. Call the poison control center if children or pets ingest the medicine by accident .

How Is Chlamydia Treated
The following are the recommended treatment regimens for chlamydia according to the Guidelines for Sexually Transmitted Diseases, released in 2015, but still considered current. Only one regimen should be chosen.
