    Put Sex On Hold During And After Chlamydia Treatment

    If youre a man who is under 25 and sexually active, its recommended that you have a chlamydia test once a year if you are not using condoms with new or casual partners.

    You could still have chlamydia even if a partner has tested negative. The only way to make sure you dont have chlamydia is to get tested yourself.

    If you have chlamydia, youll be encouraged to be tested for other STIs as you can have more than one STI at the same time.

    Getting Treated For Chlamydia And Often Gonorrhea

    If you have your own doctor, he will prescribe the antibiotics you need to treat chlamydia. If you dont have your own doctor, you can often find free or low-cost care at either a Planned Parenthood site or a community health clinic.

    In men, chlamydial infection of the lower genital tract causes urethritis and, on occasion, epididymitis. Urethritis is secondary to C. trachomatis infection in approximately 15 to 55 percent of men, although the prevalence is lower among older men.2 Symptoms, if present, include a mild to moderate, clear to white urethral discharge. This is best observed in the morning, before the patient voids. To observe the discharge, the penis may need to be milked by applying pressure from the base of the penis to the glans.

    For diagnosis of C. trachomatis infection in men with suspected urethritis, the nucleic acid amplification technique to detect chlamydial and gonococcal infections is best .4 Empiric treatment should be considered for patients who are at high risk of being lost to follow-up.

    What Happens If Chlamydia Is Left Untreated

    If left untreated, chlamydia can lead to more serious health problems.

    In people assigned female at birth, untreated chlamydia can cause pelvic inflammatory disease , a condition which can scar the fallopian tubes and lead to infertility.

    Chlamydia can also be passed on to babies during birth if the parent has the infection while pregnant.

    In people assigned male at birth, untreated chlamydia can cause epididymitis, an infection in the prostate gland, and male chlamydial urethritis.

    This is Part I of a two-part article on drug treatment of common sexually transmitted diseases. Part II, Vaginal Infections, Pelvic Inflammatory Disease and Genital Warts, will appear in the next issue of AFP.

    Several advances have been made in the treatment of sexually transmitted diseases . These advances have been incorporated into the 1998 Guidelines for the Treatment of Sexually Transmitted Diseases, published by the Centers for Disease Control and Prevention .1

    Highly effective single-dose oral therapies are now available for most common curable STDs. Single-dose regimens may be used for the treatment of chancroid, nongonococcal urethritis, uncomplicated gonococcal infections, bacterial vaginosis, trichomoniasis, candidal vaginitis and chlamydial infections.

    Improved therapies are now available for the treatment of genital herpes and human papillomavirus infections. New regimens have been approved for the use of acyclovir in the treatment of genital herpes. In addition, two new antiviral agents, valacyclovir and famciclovir , have been labeled for the treatment of genital herpes. Patient-applied therapies are now recommended for management of HPV.

    Chlamydia Treatment With Azithromycin

    Azithromycin is an antibiotic that may be used to treat chlamydia, a sexually transmitted infection affecting both men and women. The active ingredient is also called azithromycin.     Sexual assault can also increase your risk of both chlamydia or gonorrhea.

    Both STIs can be diagnosed using similar diagnostic methods. Your doctor may use one or more of these tests to ensure that the diagnosis is accurate and that the right treatment is given:

    Typical Dosing For Azithromycin

    Adults:

    • Pneumonia, strep throat, tonsillitis, and skin infections: Take 500 mg by mouth on day 1, followed by 250 mg by mouth once a day on days 2 through 5.
    • Sinus infections: Take 500 mg by mouth once a day for 3 days.
    • COPD flare-up: There are two sets of directions you can follow. You can take 500 mg by mouth once a day for 3 days, OR you can take 500 mg by mouth on day 1, followed by 250 mg by mouth once a day on days 2 through 5.
    • Chancroid, chlamydia, and infections of the urethra and cervix not caused by gonorrhea: Take 1,000 mg by mouth one time only.
    • Gonorrhea: Take 2,000 mg by mouth one time only.

    Children :

    • Middle ear infection: There are three sets of directions you can follow: Take 30 mg/kg by mouth one time only Take 10 mg/kg by mouth once a day for 3 days OR Take 10 mg/kg by mouth on day 1 and then take 5 mg/kg by mouth once a day on days 2 through 5.
    • Sinus infections: Take 10 mg/kg by mouth once a day for 3 days.
    • Pneumonia: Take 10 mg/kg by mouth on day 1 and then take 5 mg/kg by mouth once a day on days 2 through 5.
    • Strep throat and tonsillitis: Take 12 mg/kg by mouth once a day for 5 days.

The World Health Organization recently reported that certain STDs, including gonorrhea, has been growing more resilient to the antibiotics weve been discussing, such as amoxicillin, which are usually used to eradicate it.

    One of the likely causes of this advanced super gonorrhea is the fact that people often dont finish their antibiotics. Will Amoxicillin Cure Gonorrhea? Sure, most of the time. But you need to complete your medication, even if your symptoms seem to disappear. Stopping too early can allow bacteria to continue to grow and mutate. This can result in the infection coming back, or becoming far more dangerous.

    This new super gonorrhea is much harder to stop. In some severe cases, it is incurable. Most bacteria will eventually evolve, developing resistances against specific antibiotics over time. Unfortunately, that includes amoxicillin. So while amoxicillin can currently usually treat gonorrhea, it will become less effective against strains of super gonorrhea in the future.

    Is Azithromycin Better Than Doxycycline At Curing Chlamydia

    STD guidelines still favor azithromycin over doxycycline for the treatment of chlamydia. This is because of the following reasons:

    • Cure rates of azithromycin and doxycycline are similar, 97% and 98-100% respectively, according to a meta-analysis of 12 trials.
    • Azithromycin is given as a single dose, doxycycline needs to be given for seven days, either as a once-daily or twice-daily dose.
    • The dose of azithromycin can be easily supervised if need be it is much harder to supervise seven days of once daily or twice daily doxycycline treatment
    • People are more likely to take a single dose of azithromycin than finish a seven-day course of doxycycline.

    Recently, some reports have suggested that doxycycline may be more effective than azithromycin, particularly when medication adherence can be assured. There is also some concern that azithromycin may not be as effective for anogenital chlamydial infections.
